One base URL and one key for every model. The Claude ids also answer the Anthropic Messages format, so Claude Code and the Anthropic SDK work with the same key; the SDK guide has the environment variables.

What a request carries is the key, the model id and the content. The account is identified by its email address and nothing more, and request and response bodies are not stored.
